To tell the truth 'Madam' conjures memories of the quaint elderly brothel owner, a character which seems to be quite liberally litered throughout English literature. Or one of my favourite Disney films...

I don't think Ma'am was used in this time period, though, nor does it make for a very good title when combined with the name ("Ma'am [Name]" sounds wrong).
Really the choice is between Madam or Dame.
